Our Day Program supports participants to build independence, confidence, practical skills and meaningful community participation through engaging and goal-focused activities.
Independent Living Skills – cooking, meal preparation, cleaning, laundry and household skills

Social Skills – communication, friendships, teamwork and social participation

At Hope&Ability, our Support Coordination service is designed to help NDIS participants understand their plan, connect with the right supports, and get more value from their funding.
Our Support Coordinators work alongside participants, families and support networks to make the NDIS easier to navigate and ensure supports are practical, coordinated and aligned with each participant’s individual goals.
How We Can Help
We can support you to:
Understand your NDIS plan and available funding
Identify and connect with suitable service providers
Coordinate multiple services and supports
Build confidence in managing your own supports
Prepare for NDIS plan reassessments and reviews
Communicate with providers, plan managers and other stakeholders
Identify gaps in your current supports and work toward practical solutions
Respond to changes, challenges or unexpected situations
Strengthen your informal, community and mainstream supports
Build greater independence, choice and control over time
Our Approach
We understand that every participant’s situation is different. Our approach is person-centred, practical and proactive. We take the time to understand what is important to you, what is working, what is not working, and what you would like to achieve.
For participants with more complex support needs, we can also assist with coordinating multidisciplinary teams, improving communication between providers, addressing service gaps and helping everyone work toward shared goals.
Your Choice Comes First
Hope&Ability may provide a range of NDIS and allied health services. You are never required to use Hope&Ability for any other service in order to receive Support Coordination.
Our Support Coordinators will support you to consider suitable options and make your own informed decisions about the providers you choose.

Hope&Ability currently provides Support Coordination to eligible plan-managed and self-managed NDIS participants.

Habit Coaching
Build better routines. Develop practical skills. Make everyday life easier.
Habit Coaching at Hope&Ability provides practical, individualised support to help people develop routines, build consistency and strengthen the everyday habits that support greater independence.
We understand that knowing what you want to do and being able to do it consistently are not always the same thing. Difficulties with motivation, organisation, planning, remembering tasks, getting started or maintaining routines can make everyday life more challenging.
Our Habit Coaches work alongside each person to identify what is getting in the way, break goals into manageable steps and develop realistic strategies that can be practised in everyday life.
What can Habit Coaching help with?
Habit Coaching can support people with areas such as:
Establishing morning and evening routines
Building personal care and self-care routines
Planning and organising everyday tasks
Improving time management
Starting and completing tasks
Developing household routines
Building consistency with study, work or appointments
Creating healthier and more structured daily routines

How does Habit Coaching work?
Habit Coaching is tailored to the individual. Rather than simply telling someone what they should do, the coach works collaboratively with them to understand their goals, strengths, preferences and the barriers affecting their routines.
Support may include:
Setting clear and achievable goals
Breaking tasks into smaller steps
Developing personalised routines and schedules
Using visual prompts, checklists or reminders
Practising new routines in real-life environments
Identifying barriers when a routine is not working
Adjusting strategies to suit the person's needs
Building confidence and independence over time
Sessions can take place at home, in the community or online, depending on the person's goals and preferences.
A practical and person-centred approach
There is no single routine that works for everyone.
At Hope&Ability, Habit Coaching is based on the person's own goals and what is meaningful to them. Our focus is not on creating a “perfect” routine. It is about helping people develop practical systems that are realistic, sustainable and suited to their everyday life.
Where appropriate, we can also work collaboratively with family members, carers and other professionals involved in the person's support.
